Data Collected On SAE Forms And SAE Submission

Hi all

 

I was wondering whether you could tell me a bit more about the information you collect on your SAE forms? We collect site ID, DOB and pt. initials. Do you collect anything less than that?

Also, how you move the information or SAE forms about?  Do you send SAEs to reviewers via email or do you use another method? Also, how do you ask the sites to submit SAEs to you?
