At Leeds CTRU we collect DOB and initials on our CRFs to ensure participants can be accurately identified and queried with sites. We have standard processes to ensure that this data is held and managed in accordance with the DPA. Recently we have had three separate ethics committees reject applications because our collection of this data and it would be useful to understand if any other CTRUs have experienced this. Specifically:
- Do other units collect initials and DOB on the CRF? If so,
- Have any applications been rejected?
- What have you done about this?
- From which RECs have these rejections been made from.
