Wording For Patient Information Sheets For NHSD Data Applications

Would anyone be willing to share the wording you have used  in consent forms and patient information sheets to describe when the study involves requesting patient level data from NHS Digital (Hospital Episode statistics etc). Preferably examples from studies which have had success in requesting this data! The NHSD website does not seem to have such preferred text or examples so any help much appreciated!
