Capturing Reason For Change In An Audit

Hi everyone,

We are updating our audits to capture the reason for change for our in-house trial management software.  However, a discussion has grown around capturing the reason for change if a user is updating a single record but updating multiple columns for that record.

Do people have a reason for change required for each column or do you have the user make sure they complete the reason for change which would cover an entire record?
